As you may already know, we provide patients with Portal Forms to fill out before their appointments.
These forms are attached to appointment reminders that are sent a couple of days before your scheduled appointment with us.
The information required from these forms includes, checking patient details, completing an up to date medical history and if necessary, NHS forms.
Don’t worry, if you are unable to complete these forms or simply forgot to do them, we can provide you with one of our Clinipads when you arrive.
